These forms are either required for research in the VMC or will help the investigator conduct a clinical trial.

Hospital Impact Checklist	Required form for all studies taking place in the Veterinary Medical Center
Checklist for Clinical Trials - Email for a copy	Procedures at the University, College and VMC level for running a veterinary clinical trial
CIC Consent Form template	Sample consent form to be modified for particulars of the study. Contains sections listed on IACUC Appendix M.
Research Charges Form (salmon sheet)	Use this form to charge VMC procedures to the research study budget
Client Incentive Payment Form	Use this form to process payments to clients for participation in research study
