Before You Submit Samples


To assure that your sample arrives safely, please observe the following:

SAMPLE SUBMISSION CHECKLIST

  • Submit uroliths dry, do not submit in formalin, water, or urine.
  • Identify and appropriately label all containers.
  • Fill out submission form completely, including clinic address, phone and fax numbers, and e-mail address.
  • Samples can be sent via US Mail or through your preferred shipper.  For packaging instructions for non-infectious clinical samples see:  United States Postal Service Packaging Instruction 6C or contact your preferred shipper for instructions on complying with  IATA Regulation 3.6.2.2.3.6.
  • Use your complete return address.
  • Use our complete mailing address.
  • Affix sufficient postage (www.usps.com).

THE UNITED STATES POSTAL SERVICE REGARDS THE FOLLOWING AS SUSPICIOUS

Packages with:

  • No return address
  • Powdery substances on the outside 
  • Excessive or insufficient postage
  • Handwritten or poorly typed address
  • Incorrect or insufficient address
  • Excessive amount of tape
  • Strange odors or stains
  • Restrictive endorsements such as "Personal" or "Confidential"
