    2. Applicants may print the application and mail a paper copy to Karen Nelson at the address below.

  2. Official Transcripts - One from each college/university the applicant has ever attended. This includes colleges/universities abroad. 1. Professional References -  Please read the applicant instructions carefully on the reference form. Each applicant must select three evaluators. One should be from a D.V.M. who can speak to the applicant’s potential for success within the profession. The second should be an advisor or faculty member from FAMU. The third should come from an individual of the applicant’s choosing from a professional setting (work, school, volunteer organizations, etc.). Please, no friends or family members. Reference forms may be submitted to the U of M CVM in two ways:
    1. The evaluator may mail or email the completed reference form directly to Karen Nelson.
    2. The evaluator may give a paper copy to the applicant, in a sealed envelope with their signature across the flap. The applicant should then send the sealed evaluation directly to the U of M CVM. References returned by the applicant in an unsealed envelope will not be accepted.
